You will play a vital part in assessing individual needs, coordinating care plans, and facilitating the discharge process for adults, ensuring that they receive the appropriate support and resources upon leaving the hospital.
Your key responsibilities will include:
- Conducting comprehensive assessments to determine the social care needs of patients.
- Developing and implementing discharge plans in collaboration with multidisciplinary teams.
- Providing information and support to patients and their families regarding available services and resources.

To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in hospital discharge or community settings.
- Strong communication and interpersonal skills, with the ability to work effectively in a team.
