We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Hospital Team in Conwy. As a key member of our team, you will play a vital role in assessing and addressing the social care needs of patients, collaborating closely with healthcare professionals to deliver high-quality outcomes.
Your key responsibilities will include:
- Conducting comprehensive assessments of patients' social care needs.
- Developing and implementing care plans in collaboration with multidisciplinary teams.
- Providing support and guidance to patients and their families throughout the hospital stay.
- Facilitating discharge planning and ensuring appropriate follow-up services are in place.
- Maintaining accurate records and documentation in accordance with legal and organizational standards.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., BA/BSc in Social Work or equivalent).
- Current registration with Social Care Wales.
- Experience working in adult social care, preferably within a hospital setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage multiple cases effectively.
