We are seeking a dedicated and compassionate Social Worker to join our Adults Assessment and Care Delivery team in Liverpool. This role is pivotal in providing high-quality support and assessment for adults in need, ensuring they receive the care and services necessary for their well-being. You will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and deliver effective interventions.
Key responsibilities include:
- Conducting comprehensive assessments of individuals' needs and circumstances.
- Developing and implementing personalized care plans in collaboration with clients and their families.
- Providing ongoing support and monitoring of client progress, adjusting care plans as necessary.
- Facilitating access to resources and services, including community support and financial assistance.
- Maintaining accurate and up-to-date case records and documentation in compliance with regulatory standards.
- Working closely with other professionals, including healthcare providers, to ensure holistic care delivery.
To be successful in this role, candidates must possess the following qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
- Registered with Social Work England.
- Strong understanding of adult safeguarding and mental health legislation.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients.
- Proficiency in assessment and care planning processes.
- Demonstrated ability to work independently and as part of a team.
