We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Learning Disabilities (LD) Team in Leeds. This is an exciting opportunity for a motivated individual to make a real difference in the lives of adults with learning disabilities, ensuring they receive the support and services they need to thrive in their communities.
As a Qualified Social Worker in our LD Team, you will be responsible for:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and develop tailored support plans.
- Working collaboratively with individuals, families, and multi-disciplinary teams to ensure the delivery of high-quality care and support services.
- Advocating for the rights and best interests of service users, promoting independence and inclusion in community life.
- Monitoring and reviewing care plans regularly, making necessary adjustments to ensure effective service delivery.
- Keeping accurate and up-to-date records in accordance with legal and organizational standards.
The ideal candidate will possess the following qualifications and skills:
- A recognized social work qualification (e.g., DipSW, CQSW, or equivalent).
- Registered with Social Work England.
- Proven experience in working with adults with learning disabilities or related fields.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
- Excellent organizational skills and the ability to manage a complex caseload effectively.
