Social Worker - Children with Disabilities Team

Caritas Recruitment is currently recruiting for an experienced Social Worker to join a dedicated Children with Disabilities Team based in Kings Hill.
This is an excellent opportunity to make a real difference to the lives of children and young people with disabilities and their families, supporting them to access the services and support they need while safeguarding their wellbeing and promoting positive outcomes.
Key Responsibilities
- Manage a varied caseload of children with disabilities and their families.
- Undertake assessments and develop plans to identify needs and appropriate support arrangements.
- Work in partnership with children, families, carers, education, health professionals and other agencies to deliver effective interventions.
- Facilitate access to community resources, direct payments and commissioned services.
- Ensure support plans are outcome-focused, person-centred and compliant with statutory duties.
- Identify and respond to safeguarding concerns, undertaking investigations and appropriate actions in line with policy and procedure.
- Promote self-determination and ensure advocacy services are available where required.
- Maintain accurate case records and reports in line with legislative and organisational requirements.
Requirements
- Qualified Social Worker with Social Work England registration.
- Experience working with children and families within a statutory setting.
- Previous experience within a Children with Disabilities Team is desirable.
- Strong assessment, safeguarding and care planning skills.
- Ability to work effectively within a multi-agency environment.
