We are seeking a dedicated and passionate Qualified Social Worker to join our Community Wellbeing Team in Herefordshire. This role is pivotal in promoting the welfare and wellbeing of adults within our community. As a member of our team, you will work collaboratively with individuals, families, and various stakeholders to ensure that our clients receive the support they need to lead fulfilling lives.
Your key responsibilities will include:
- Conducting comprehensive assessments of adults in need of support, identifying their strengths and needs.
- Developing and implementing tailored care plans in collaboration with clients and their families.
- Providing ongoing support and intervention, ensuring that the wellbeing of clients is prioritized.
- Working closely with multidisciplinary teams, including healthcare professionals and community organizations.
- Maintaining accurate and up-to-date case records and documentation in compliance with relevant legislation.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in community settings.
- Strong assessment and analytical skills, with the ability to develop effective support plans.
- Excellent communication and interpersonal skills, with a compassionate approach to client interactions.
