Key responsibilities include:

- Working closely with multidisciplinary teams, including health professionals, to deliver integrated services.
- Maintaining accurate and up-to-date case records in accordance with local policies and statutory requirements.
- Participating in training and professional development opportunities to enhance your skills and knowledge.
To be considered for this position, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registration with Social Work England.
- Strong knowledge of relevant legislation and best practices in adult social care.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients.
- Demonstrated experience in assessment, care planning, and safeguarding vulnerable adults.
- Ability to work independently and as part of a team, managing a caseload effectively.
