As an Adult's Social Worker, your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and aspirations of young adults with disabilities.
  • Developing and implementing support plans tailored to individual needs, ensuring a person-centered approach.
  • Coordinating with various stakeholders, including families, healthcare providers, and community organizations, to facilitate the transition process.
  • Providing ongoing support and guidance to young adults and their families throughout the transition to adulthood.
  • Maintaining accurate case records and documentation in compliance with local and national standards.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
  • Current registration with Social Work England.
  • Experience in working with young adults with disabilities or in a similar field.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
  • Excellent organizational skills and the ability to manage a caseload effectively.
