In this role, you will play a crucial part in supporting young individuals as they navigate the transition from adolescence to adulthood, ensuring they receive the guidance and resources needed to thrive in their new phase of life.
As a Senior Practitioner, your key responsibilities will include:
- Developing and implementing personalized transition plans for young adults, focusing on their unique needs and aspirations.
- Providing expert advice and support to social workers and other team members, fostering a collaborative environment.
- Conducting assessments and evaluations to identify risks and opportunities for young adults in transition.
- Building strong relationships with young adults, their families, and external agencies to facilitate effective support systems.
- Monitoring and reviewing progress, ensuring that all interventions are effective and aligned with best practices.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Significant experience working with young adults, particularly in transition settings.
- Strong knowledge of relevant legislation, policies, and best practices in social work.
- Excellent communication and interpersonal skills, with an ability to engage and motivate young people.
