Key responsibilities include:

To be successful in this role, candidates must possess the following qualifications and skills:
- A recognized degree in Social Work and current registration with Social Work England.
- Proven experience in hospital discharge planning or a related field.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent problem-solving and organizational skills, with a keen attention to detail.
