An experienced and dedicated Adult Social Worker is required to join a busy Social Care Team in Blackburn. This is an opportunity to make a meaningful difference to adults and their families by delivering person-centred assessments, care planning and ongoing support. You will work with individuals who may have a range of needs, including physical disabilities, learning disabilities, mental health conditions, sensory impairments and age-related support requirements.
Your responsibilities will include completing strengths-based assessments under relevant social care legislation, identifying eligible needs and managing a varied caseload. You will develop, review and monitor care and support plans, promote independence and wellbeing, and ensure that individuals are fully involved in decisions about their lives. The role will also involve safeguarding adults, undertaking risk assessments, coordinating packages of care, arranging appropriate services and working closely with health professionals, families, providers and other partner agencies. Accurate recording, report writing and effective case management will be essential, alongside the ability to manage competing priorities in a fast-paced environment.
Applicants must hold a recognised Social Work qualification and current registration with Social Work England. Previous experience in adult social care, statutory assessment and safeguarding is desirable, together with a sound understanding of the Care Act and associated legal and professional frameworks. You should be confident working autonomously while contributing positively to a multidisciplinary team, with strong communication, analytical and organisational skills. A compassionate, resilient and solution-focused approach is essential, as is a commitment to equality, dignity, strengths-based practice and positive outcomes. This role may require travel across the local area, so the ability to travel between locations is advantageous.
