An experienced Adult Social Worker is required to join an Urgent Response Team, supporting adults and their families during periods of crisis or rapidly changing need. The role involves completing timely assessments, identifying immediate risks, arranging proportionate care and support, and helping people remain safe and independent in their own homes wherever possible. You will work with individuals presenting with complex physical health, mental health, safeguarding, or social care needs, often within challenging timeframes.
Your responsibilities will include undertaking strengths-based assessments, developing and reviewing care and support plans, coordinating short-term interventions, and arranging appropriate packages of care, equipment, reablement, or alternative support. You will contribute to hospital discharge and admission avoidance pathways, respond to safeguarding concerns, and apply relevant legislation and policy, including the Care Act, Mental Capacity Act, and safeguarding procedures. Effective partnership working with health professionals, care providers, families, carers, and other agencies will be essential to achieving positive outcomes.
You must hold a recognised Social Work qualification and current registration with Social Work England. Previous experience in adult social care, urgent response, hospital discharge, community services, or a similarly fast-paced environment is highly desirable. You should have sound knowledge of strengths-based practice, risk assessment, safeguarding, capacity and consent, and person-centred care planning. Excellent communication, professional judgement, organisation, and record-keeping skills are required, along with the ability to prioritise competing demands and work confidently both independently and as part of a multidisciplinary team. A valid driving licence and access to suitable transport may be required, depending on service needs.
