We are seeking a dedicated and compassionate Adult Social Worker to join our Independent Supported Living team in Middlesbrough. As a vital member of our team, you will be responsible for assessing needs, developing care plans, and providing ongoing support to individuals in their own homes.
Your key responsibilities will include:
- Conducting thorough assessments of clients' needs and risks.
- Developing, implementing, and reviewing personalized care plans.
- Collaborating with multi-disciplinary teams to coordinate services.
- Providing guidance and support to clients and their families.
- Ensuring compliance with all relevant legislation and best practices.
The ideal candidate will possess the following qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
- Current registration with Social Work England.
- Proven experience in adult social work, particularly in supported living environments.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
