We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Life After Stroke Service based across Cornwall. This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.

Location: Home‑based in Mid Cornwall and West Cornwall, with frequent travel required for team meetings and client visits.

Hours & Contract: Part‑time, 24 hours per week. Two posts available – one maternity cover contract. Fixed‑term contract until 31 March 2027.

The Role: The Stroke Support Coordinator will support new stroke survivors and their carers from hospital discharge into the community, provide personalised information, advice and support, help survivors make informed lifestyle changes to prevent further strokes, and work collaboratively with NHS colleagues and community organisations.

Qualifications: Experience working with people in a caring profession or caring capacity. Ability to maintain accurate records and use IT skills and database systems. Effective caseload management and the ability to use a flexible working approach. Excellent interpersonal skills to work with a diverse range of people.

Requirements: Extensive travel across Cornwall to visit people at home and in community settings. Candidates must be UK residents with the right to work in the UK.
