We are looking for an innovative, passionate, and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Service based in Bedford.
This is an exciting opportunity to work with stroke survivors and their families to support them following stroke.
Location
Home base in Bedfordshire, with frequent travel for community visits, team meetings, and other work‑related meetings.
Hours and Contract
Part‑time, 28 hours per week. Services are contracted with funding available until 31 March 2027.

Role
Reporting to the Service Delivery Coach in the East of England, you will support stroke survivors to identify their goals and use a person‑centred approach to maximise their recovery and improve communication.
Key Responsibilities
- Support newly diagnosed stroke survivors and their carers from hospital discharge into the community.
- Support a diverse caseload, including anyone experiencing communication or cognitive difficulties.
- Provide support via face‑to‑face visits, telephone calls, emails or letters, and digital methods such as video calls.
- Use a person‑centred and person‑first approach to deliver specific, tailored information, advice, and support to stroke survivors and carers.
- Empower stroke survivors to make informed lifestyle changes that help them live well after stroke.
- Collaborate with other health and social care professionals across the stroke pathway to ensure high‑quality support.
About You
We are looking for someone who:
- Has experience working in a caring profession or other caring capacity.
- Can maintain accurate records using IT and database systems.
- Has effective caseload management skills and can work flexibly.
- Exhibits excellent interpersonal skills and can work with a diverse range of people.
This role requires extensive travel across the local service area and further afield for team and locality meetings. Candidates must be able to demonstrate that they can meet this requirement.
