Job Description: We are looking for a proactive and compassionate Care Manager to oversee our domiciliary care services. The Care Manager will be responsible for coordinating care plans, managing a team of care providers, and ensuring the well-being of our clients receiving home-based care. Key Responsibilities:

1. Develop individualized care plans for clients receiving domiciliary care

3. Conduct regular assessments of clients’ needs and adjust care plans accordingly

Qualifications:

1. Degree in Health & Social Care or a related field
