This is a full-time on-site role for a Care Manager, located in Fleet. The Care Manager will be responsible for managing and coordinating service user’s care plans, conducting assessments, and ensuring the highest standards of care are maintained. The role involves providing support and guidance to service users and their families, collaborating with healthcare professionals, and ensuring compliance with healthcare regulations.
