About the Role
As a Field Care Manager, you will play a vital role in ensuring our service users across Sutton receive safe, effective, and person-centred care. You will support our care teams in the field, complete care assessments, develop care plans, and monitor service delivery to maintain our excellent quality of care.
Key Responsibilities
- Conduct initial assessments and regular reviews of care plans.
- Supervise and support care staff in the community.
