As a Care Assistant within our Community Recovery Services team, you will play a vital role in supporting individuals to regain confidence, independence, and daily living skills following illness, injury, or hospital admission. Working within the community, you will provide person-centred care that promotes dignity, choice, and recovery.
This role involves supporting people in their own homes to achieve agreed outcomes and rebuild independence, rather than creating long-term dependency.
Principal responsibilities- To assist individuals with personal care tasks, including washing, dressing, toileting, and maintaining hygiene in a respectful and dignified manner.
- To support clients with mobility, including the safe use of aids, equipment, and rehabilitation techniques as directed by health professionals.
- To promote independence through encouragement, motivation, and active participation in daily routines such as meal preparation, household tasks, and personal goal setting.
- To provide support with medication prompts in accordance with individual care plans.
- To observe, record, and report any changes in an individual's health, wellbeing, or circumstances to the Recovery Support Coordinator or Community Team Lead.
- To work collaboratively with occupational therapists, physiotherapists, and nurses to deliver personalised recovery plans.
- To encourage engagement in social and therapeutic activities that support emotional and physical wellbeing.
- To assist with light domestic duties where required, ensuring a safe and comfortable home environment.
- To maintain accurate records and contribute to reviews, ensuring that care remains responsive and effective.
- To adhere to all company policies, procedures, and safeguarding standards at all times.
