We are looking for a skilled, compassionate, and motivated Care Home Assistant Practitioner (CHAP) to join our dedicated team. This role is ideal for an experienced healthcare professional who has completed accredited CHAP training and is committed to delivering safe, effective, and person-centred care throughout the night.

Responsibilities

  • Support Registered Nurses in delivering exceptional and person-centred care.
  • Safely administer medication and undertake clinical procedures such as wound care, catheter care, and blood sugar monitoring (following competency sign-off).
  • Assist with PEG/PEJ care, nursing observations, and other delegated clinical interventions.
  • Record vital signs, food and fluid intake, and maintain clear, accurate, and confidential care records.
  • Provide personal care and assist residents with mobility, repositioning, and activities of daily living.
  • Promote resident dignity, independence, wellbeing, and choice at all times.
  • Support, guide, and mentor care staff to enhance practice and maintain high-quality standards.
  • Contribute to developing, reviewing, and implementing care plans and risk assessments.
  • Maintain a safe working environment and comply with all health & safety requirements.
  • Uphold confidentiality, professionalism, and company values in every aspect of your work.

Qualifications

  • Accredited CHAP qualification (Care Home Assistant Practitioner training completed).
  • NVQ Level 3 in Health and Social Care, or an equivalent qualification.
  • Minimum of 12 months' experience in a similar healthcare role.
  • Competence in administering medication and delivering personal care.
  • Experience working night shifts and comfortable with the responsibilities they involve.
  • Good understanding of the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS).
  • Ability to support residents with complex health needs, including tracheotomy and PEG/PEJ care.
  • Strong communication skills, attention to detail, and a caring, professional attitude.
  • Enhanced Disclosure and Barring Service (DBS) check is required for this role.
