Description: Care Coordinator About us Care Focal is a CQC Registered Care provider based in Erith and East London, with operational areas mainly across Bexley, Dartford, Greenwich, Bromley, Lewisham, Stratford, Bow, Barking, Medway and Essex.
We provide high-quality home care and community services to vulnerable Adult and Children.
Summary of role: Care Coordinator The Care Coordinator is to generate and promote a caring and supportive environment for Care Focal’s clients by implementing very high standards of professional care and support.
To manage the staff team, supporting their development, skills and abilities.
Principal responsibilities: Care Coordinator The Care Coordinator will provide good service to clients by ensuring that prior to each service commencing, a risk assessment with the client, and/or their chosen representatives, has been completed including what the client needs and would like to achieve from their care and support.
The Care Coordinator will implement quality management and improvement systems.
Effectively manage complaints and incidents.
Carry out investigations relating to the quality of the service and use findings to make improvements.
