The predicted salary is between 13 - 16 £ per hour.

This role provides a statutory service within the Council to ensure compliance with the main provisions of the Leaving Care Act 2000 and meets performance indicators expected. The aim is to provide a Personal Advisor service to meet the needs of young people who have left or are about to leave the care of the Authority and who are eligible for a service under s24 of the Children Act 1989 or under the provisions of the Children (Leaving Care) Act 2000 and Social Services and Wellbeing Act 2014 (especially Part 6) and the relevant Regulations.

Description of Principal Duty or Responsibility:

  • To be the allocated personal advisor and to initiate a professional relationship with care leavers aged 15.9 months plus.
  • Completion of My Pathway Plan by 16 with the young person.
  • To provide advice to the young people, carers, and staff on benefit entitlements.
  • To attend the young person's Pathway Plan Review and Looked After Children Reviews.
  • To provide advice on Education, Employment, and Training opportunities.
  • To support the preparation of the Pathway Plan.
  • To liaise with the case holding social worker for eligible and relevant young persons in the implementation of the pathway plan and assessment.

Qualifications:

  • Minimum of 5 Grade C GCSE passes (or equivalent) including Maths and English, or equivalent relevant experience demonstrated.
  • NVQ level 3 in Health and Social Care or equivalent.

Specialist Knowledge:

  • Good working knowledge of the Children’s Act 1989 and Children (Leaving Care) Act 2000.
  • Experience of working with young people with complex issues.

Practical and Intellectual Skills:

  • Ability to communicate in Welsh.
  • Good communication skills, both oral and written.
  • Computer skills.

Personal Attributes:

  • Empathy with problems of homelessness.
  • Ability to work as part of a team.
  • Ability to work on own initiative.
  • Ability to work to statutory deadlines.
  • Ability to manage time to meet own workload.

Personal Circumstances:

  • Ability to travel across the County Borough to carry out work (e.g. attend meetings / visit clients or work sites).
