The Health Visiting service is part of the wider Children’s Health services providing community-based services for children and young people aged 0-5 years. The role supports the delivery of the Healthy Child Programme (HCP) 0‑5 to a defined population within the 0‑5 thriving Hub, using evidence‑based, needs-led service models across a variety of community settings including child health clinics, homes, early years settings and children’s centres.

Key Responsibilities

  • Work within the scope of competence and conduct health assessments and developmental screenings for children.
  • Provide early intervention and support for developmental concerns.
  • Offer advice and support to parents on child health, nutrition, and development.
  • Implement care plans for children with identified needs as part of the Family Help approach.
  • Assist in planning, delivery and evaluation of programmes of care aimed at improving physical and emotional health, such as obesity/healthy eating, parenting programmes, children’s and parental emotional health, health education and promotion activities.
  • Support qualified professionals delegated to assess the needs of children and families through individual health reviews, monitoring of growth, and addressing emotional health needs.
  • Lead and coordinate health promotion activities within the nursery setting.
  • Provide training and development to nursery staff on health‑related topics.
  • Liaise with other health professionals (GPs, paediatricians, social workers) to ensure integrated care.
  • Maintain accurate and up‑to‑date records of all interactions and interventions.
  • Promote multi‑agency working and support this by attending meetings and sharing information (e.g., Child Protection processes, Family Help support, safeguarding).
  • Safely maintain confidential, professional records in line with information governance policy.
  • Participate in clinical and other audits as required and in safeguarding supervision per the supervision framework.
  • Support the team by providing learning opportunities for students.
