The Community Mental Health Service supports adults living with severe and enduring mental health difficulties in the community.
As a Band 6 Community Mental Health Nurse, you will:
- Deliver psychosocial, evidence-based interventions, including solution-focused work, motivational interviewing and recovery-focused care.
- Work proactively with individuals experiencing relapse, using a strengths-based approach to reduce dependency on mental health services.
- Develop and communicate recovery care plans and risk management plans, ensuring information is shared appropriately with relevant partners.
- Liaise with a wide range of stakeholders, including voluntary sector partners, primary care, housing providers and safeguarding services, to provide holistic and joined-up care.
- Use your professional expertise to support colleagues through supervision, reflective practice and contributing to team development.
- Maintain accurate, up-to-date clinical records on Trust systems and ensure adherence to Trust policies, professional standards and governance requirements.
- Participate in regular supervision and appraisal, and keep up to date with current research, policy and best practice in mental health care.
- Actively promote the Trust's values of Kindness, Respect and Teamwork in all aspects of your role.
