We have an exciting part‐time opportunity for a staff member to work in the Runcorn area, covering several different GP surgeries within the working hours. The post is 2 days a week for a total of 15 hours, with core working hours of 9 am–5 pm (excluding bank holidays). Applicants must be able to travel between the surgeries.

The successful applicant will work in a Primary Care setting, offering specialist mental health assessment to patients with moderate to severe mental health conditions. The role spans the interface between Primary Care and Secondary Care Mental Health Services, providing advice, support, and assessment to ensure timely access and appropriate treatment.

Responsibilities:
  • Provide day‑to‑day clinical service, including telephone triage screening assessments to gauge risk/priority and determine the appropriate action (treatment, sign‑posting or referral).
  • Conduct face‑to‑face assessments, establishing risk, determining treatment plans and engaging patients in a partnership approach.
  • Support adherence to prescribed treatments, monitor mental health medication and consider any physical health interactions.
  • Deliver short‑term interventions and support under GP guidance, ensuring monitoring and line‑with practice standards.
  • Provide and receive complex information with patients, families and carers, actively supporting as appropriate.
  • Collate and organise information on local resources for patients, facilitate sign‑posting to relevant services and, when necessary, undertake very short‑term interventions.
  • Evaluate outcomes and maintain accurate clinical data.
  • Understand and apply Quality Outcome Framework data.
  • Undertake joint assessment work with PCN colleagues.
  • Offer advice to and obtain advice from the Primary Care Team on interventions and ongoing care management.
  • Ensure systems and pathways enable smooth transition between primary and secondary services.
  • Participate in quality improvement activities, audits, significant event reviews, and clinical meetings.
  • Support establishment of partnerships and networks with stakeholders across PCN and secondary mental health services.
  • Develop and implement joint governance and clinical forums with PCN members.
Qualifications:
  • Registered with the NMC as a Mental Health Nurse.
  • Experience in community practice and a clear understanding of the role.
  • Capability to work autonomously and as part of a multidisciplinary team.
