The post holder will play an integral role in supporting the Lead Homecare Pharmacist and Lead Pharmacist of Medicines Formulary and High-Cost Drugs to ensure the timely, safe, and efficient delivery of a high-quality, patient-focused homecare medicines service whilst also supporting medicines optimisation across the Trust.
Support and work closely with the Lead Homecare Pharmacist to deliver a safe, high-quality pharmacy homecare service in line with national standards and policies.
- Manage and coordinate day-to-day homecare operations, including staff supervision, workload organization, resolving medication/service issues, and maintaining high patient care standards.
- Act as a key contact between the Trust, homecare providers, pharmacists, clinical teams, suppliers, and patients, handling queries and supporting communication across services.
- Contribute to service improvement through developing procedures, monitoring KPIs, handling complaints/incidents, producing reports, and participating in audits, reviews, and meetings.
- Support staff training, recruitment, and development, while also assisting with medication coordination, ordering, and accuracy checking to ensure timely patient treatment.
- Support medicines optimisation by carrying out audits, producing reports and participating in service review to improve patient care and efficiency.
