We are looking for a confident, highly motivated, experienced Clinical Sister or Charge Nurse to work with one of our integrated community nursing teams. You will work alongside registered nurses and other health and social care professionals and will support staff to provide assessment, care and treatment to adults and older people living at home with complex, long‑term physical health needs.

Responsibilities
- Work autonomously within a dedicated community nursing team alongside registered nurses and other health and social care professionals
- Provide assessment, care and treatment to adults and older people living at home with complex, long‑term physical health needs
- Deliver evidence‑based, high‑quality care to patients with life‑limiting illnesses and support their families
- Demonstrate innovative clinical management and leadership skills, initiating and mentoring professional development for team members
- Support the Locality Lead in the development of the Community Nursing Service
Management and Leadership
- Support the community nursing service in providing high‑quality, individualised end‑of‑life care to patients and their carers at home to improve quality of life and provide a safe alternative to inpatient care when appropriate.
- Develop communication skills to support patients and families in highly emotional and distressing circumstances when discussing sensitive information.
- Contribute to service development and coordinate its implementation.
- Supervise qualified and unqualified staff, identify training needs, and facilitate continued professional development through the PDR/1:1 process.
- Ensure effective learning experiences and opportunities for students and registered practitioners through mentorship, preceptorship, clinical supervision, and provision of learning opportunities.
Care Delivery
- Take responsibility and accountability for assessment of care needs, development, implementation and evaluation of care to ensure the highest quality of care.
- Provide care for patients and families from diverse cultural, religious, and ethnic backgrounds.
- Demonstrate analytical and high‑level judgment when interpreting and performing acute or complex patient assessments and care planning.
- Maintain accurate, up‑to‑date nursing records, patient confidentiality, and required statistical records.
- Work with other professionals to promote multidisciplinary working, appropriate skill transfer, and the creation and use of care pathways.
