As a Care Coordinator, you will play a vital part in ensuring the smooth delivery of our care services. You'll be responsible for scheduling care visits, communicating with carers and service users, and ensuring compliance with regulatory standards. This is a fast-paced role that requires excellent communication, attention to detail, and a passion for supporting others.
Key Responsibilities
- Create and maintain weekly care rotas, ensuring all visits are allocated appropriately.
- Act as a main point of contact for care staff, service users, and families.
- Respond promptly to changes, emergencies, and staff absences.
- Ensure records are kept up to date.
- Support ongoing supervision of care workers.
- Maintain compliance with CQC standards and organisational policies.
- Work collaboratively with local authorities, healthcare professionals, and external partners.
- Participate in on-call rota duties as required.

Essential skills and experience:
- Previous experience in care coordination or similar role.
- Strong knowledge of domiciliary care or health & social care settings.
- Excellent planning, time‑management, and problem‑solving skills.
- Good IT skills (care management software experience desirable).
- Understanding of CQC regulations and safeguarding principles.
- Must be a driver with a vehicle.
