The post holder is suitable for a Registered Nurse, Allied Health Professional or Paramedic with a broad general experience as a qualified professional.
The Care Coordination Centre is responsible for the triaging of referrals, telephone calls and the daily management of the Centre between the hours of 08:00 until 20:00 seven days a week.
The Coordination Centre will be responsible for triaging referrals on to unplanned caseload (Rapid Response and Hospital at Home), identifying the most appropriate professional to see the patient, allocating visits and liaising with clinical staff.
The candidate will be responsible for signposting patients and/or their carers/relatives to the provision of accurate healthcare information, self-care/health improvement advice or, where applicable, referral to the appropriate level and location of care, to meet the holistic and therapeutic requirements of the patient/carer.
The candidate is responsible for the management and clinical supervision of junior staff and the facilitation of data collection, audits, and quality improvement to the service.
