Continuity of care is key for all individuals, but especially for those at the end of life. Integrated Care Records (ICR), also known as Electronic Palliative Care Coordination Systems (EPaCCS), aim to support people approaching the end of life to receive consistent care, in line with their wishes, that is coordinated across multiple services.
An end of life integrated care record has been created with the digital team on the platform 'Graphnet'. This post will involve the planning, creation and delivery of awareness and education supporting the implementation of this End of Life Integrated Care Record across Bath and North East Somerset, Swindon and Wiltshire. It will traverse primary, secondary, community and private/independent services, including engagement with clinical and non-clinical environments. The clinical and non-clinical impact of the process will be measured as part of the project, and the post holder will be expected to have a significant role in planning and carrying this out.
Applicants will be self-motivated, able to meet deadlines and engage with, learn from and educate patients, families and staff from different professional disciplines. The post is part time for 2.5 days a week (0.5 WTE) for a fixed term contract of 3 years. The role will be supported by the lead palliative medicine consultant and will involve travelling to a variety of sites with flexibility for aspects to be done from home.
Responsibilities:- Under supervision, plan and carry out the required communication and education around the End of Life Integrated Care Record and digital ReSPECT to all healthcare organisations across Wiltshire. This may include some work in Swindon and BANES.
- Work with external stakeholders to achieve this and with other members of the ICR team to ensure a cohesive and supportive approach.
- Clinical Governance and Audit: ensure work done is with the highest integrity. Opportunities to participate in audit and QI work assessing the roll out.
