An exciting opportunity for a motivated and compassionate General Nursing Assistant with a keen interest in stoma care to join our Stoma Service. In this role you will support and assist stoma patients following stoma formation, teaching patients to become independent. You will work closely with Stoma specialist nurses to deliver individualised patient care plans and ensuring a smooth transition from hospital to community settings. This is a varied role, and you will work across MCHT and CCICP.
Responsibilities
- Assist the Specialist Stoma Nurses in the delivery of day to day education for stoma patients.
- Provide dietary and fluid advice.
- Stoma products review within patient homes.
- Support patients, families, and carers to achieve their optimum health and independence.
- Act as a role model for excellence in the delivery of bladder and bowel care and the MCHFT Trust ‘Values and Behaviours’.
- Work within and promote CCICP/MCHFT policy and procedures.
- Collaborate with the multidisciplinary team/agencies to ensure that patient needs are met.
- Involve patients, relatives/carers in the delivery of care, providing reassurance and support to assist their understanding and co-operation.
- Prepare the clinical area for safe practice by ensuring cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards.
- Collect, collate and record information, maintaining accurate patient records as per trust standards.
- Participate in the delivery of patient care, following agreed plans, policies, pathways and procedures relevant to job role.
- Recognise and respond appropriately to urgent and emergency situations.
- Recognise any changes in patients' condition or stoma, ensuring it is reported to the relevant staff in a timely manner.
- Enable service users wherever possible to achieve independence in looking after their health needs.
- Provide written information to support this and digital information to those users with technology.
- Use and maintain equipment as trained to do so and be responsible for the maintenance of the equipment ensuring it is in good working order.
- Ensure stock in clinical areas is ordered to demand, stock is rotated and is stored appropriately.
