Main Duties and Responsibilities

  • Clinical Practice and Leadership
    • Deliver expert nursing assessment, intervention, and management for patients referred for specialist heart failure care.
    • Act as a key point of contact for patients, families, carers, and staff, offering clinical advice, education, and support.
    • Work autonomously within scope of practice, collaborating with multidisciplinary teams to ensure high-quality, evidence-based care.
    • Lead on the development and implementation of clinical guidelines, protocols, and care pathways.
    • Promote safe, effective clinical practice and contribute to the achievement of organisational objectives.
  • Governance and Risk Management
    • Maintain compliance with Trust policies, procedures, and clinical guidelines.
    • Participate in and lead risk assessments, ensuring incidents and near misses are reported, investigated, and managed appropriately.
    • Lead reflective discussions and disseminate learning from incidents to improve practice.
    • Support continuous quality improvement and assurance through audits, data analysis, and service evaluations.
  • Education, Training, and Staff Development
    • Act as a clinical educator and mentor, supporting the development of nursing and multidisciplinary staff.
    • Deliver formal and informal education sessions, seminars, and training programmes aligned with clinical standards and protocols.
    • Ensure a supportive learning environment for pre- and post-registration learners.
    • Contribute to the development of education and training standards within the specialist field.
  • Service Development and Innovation
    • Contribute to and lead service improvement initiatives, audits, and accreditation processes.
    • Undertake literature reviews and support the development of policies, procedures, and guidelines.
    • Participate in multidisciplinary meetings, ward rounds, and case discussions to enhance integrated care.
    • Promote collaborative working across organisational and professional boundaries to improve patient pathways.
  • Resource and Workload Management
    • Manage own caseload and prioritise workload effectively, identifying capacity and escalating pressures as needed.
    • Support the development of the heart failure nursing team in response to changing patient needs and service demands.
    • Contribute to the development and use of information systems for clinical audit, reporting, and service planning.
    • Maintain a safe, clean clinical environment and ensure equipment is used and maintained appropriately.
  • Patient Experience and Communication
    • Lead on initiatives to capture and respond to patient feedback, ensuring concerns and complaints are addressed promptly.
    • Chair and participate in multidisciplinary forums, ensuring actions are recorded and followed up.
    • Foster open, effective communication within the team and with external stakeholders.
    • Promote patient and public involvement in service planning and evaluation.
  • Research and Evidence-Based Practice
    • Apply research findings to improve clinical practice and patient care.
    • Support or lead clinical trials and research projects within the specialist area.
    • Identify opportunities for innovation and contribute to the development of new models of care.

    • Maintain NMC registration and engage in ongoing professional development and revalidation.
    • Participate in the appraisal process and support the appraisal and development of others.
