An exciting opportunity has arisen for a band 6 nurse to join the SNOP team (Specialist nurses for older persons). The SNOPs are specialist nurses based throughout the trust including Acute care, DME, ED and the frailty unit. The successful candidate will holistically assess our older patients at the front door, aiming to reduce the risk of deconditioning while waiting and support rapid discharge.

Key Responsibilities

  • Assess, plan, implement and evaluate care for older frail patients, offering advice and support to ward staff, patients and their families.
  • Provide expert advice and clinical practice, performing skills required for patient assessment such as catheter insertion, cannulation, ECGs etc.
  • Participate in the education and development of other staff members.
  • Work collaboratively with clinical colleagues to develop services and quality of care delivered.
  • Ensure the management plan following CGA is commenced for all relevant patients following assessment, aiming to complete within 1 hour of admission.

Qualifications

  • Minimum of 2 years qualified or have frailty experience.
  • Will receive specialist training and support from the MDT team.
