Qualified social workers are invited to join our Integrated Services for adults with disabilities at the Millennium Centre in Leasowe. As a person‑centred practitioner you will support adults with complex needs and their carers to achieve their desired outcomes.

Key Responsibilities

  • Carry out Care Act assessments and complete mental capacity assessments.
  • Develop personalised support plans following asset and strength‑based approaches.
  • Lead safeguarding enquiries and manage related risk and reporting.
  • Work collaboratively with the wider multidisciplinary team to ensure coordinated care for service users.

Qualifications and Experience

  • Registered social worker with Social Work England (SWE).
  • Experience in Care Act assessment, support planning and safeguarding.
  • Good understanding of asset‑based and strength‑based assessment approaches.
