We are recruiting a 26.5 hour permanent Social Care Assessor for the Winsford Community Social Care team at Wyvern House, Cheshire West and Chester Council.

Responsibilities

  • Support adults to develop creative and innovative solutions to meet their personal care and support goals.
  • Work with local people to understand their care needs and identify available options.
  • Ensure people are connected to the support services and community assets they require.
  • Provide person‑centred social care in line with the Care Act in collaboration with social workers and occupational therapists.
  • Co‑ordinate multidisciplinary working and maintain an integrated approach.
  • Plan and organise support activities effectively and adapt positively to changes.
