An exciting opportunity has arisen within the Shrewsbury & Telford Hospital for a Senior Heart Assessment Team Sister/Charge Nurse managing heart failure and cardiac rehabilitation patients. The successful applicant will be a qualified NMC registered Nurse with proven post registration experience in Cardiology at Band 6 level or above. This part time permanent position is at The Royal Shrewsbury Hospital. Within this role there may be the need to work cross site.

Band 6 to Band 7 developmental role would be considered and there will be the expectation that the Physical Assessment Course and Non Medical Prescribing Course will be undertaken within 2 years of appointment to the Band 6 developmental role and will allow automatic appointment to Band 7, on completion of this and demonstration of the essential criteria for the Band 7 role.

Working as an autonomous practitioner, the post holder will provide a comprehensive highly specialist service to patients with heart failure and cardiac rehabilitation needs participating in the delivery of patient‑centred care. The post holder will act as an expert and resource for members of the multi‑disciplinary team supporting the clinical assessment and management of all patients with heart failure and helping to ensure a comprehensive, patient‑centred, integrated service is delivered.

The post holder will be proactive in optimising evidence‑based pharmacological and non‑pharmacological treatments in an effort to reduce symptoms, improve quality of life and reduce hospital admissions and length of hospital inpatient stay. Within the role, the post holder will have responsibility for providing ongoing assessment support and lifestyle advice to cardiac patients on an inpatient and outpatient basis through one‑to‑one consultations or group sessions and will have responsibility for leading junior team members in the delivery of a comprehensive cardiac rehabilitation exercise and education programme.
