Overview
Clinical Support: Assist in medication reviews. Reconcile medicines following hospital discharge and outpatient clinic appointments. Provide advice to clinicians on suitable formulary alternatives for out of stock medication, the Pharmacy First Consultation Service and guidance on medications which may be purchased over the counter.
Responsibilities
- Clinical Support: Assist in medication reviews; reconcile medicines following hospital discharge and outpatient clinic appointments. Provide advice to clinicians on suitable formulary alternatives for out of stock medication, the Pharmacy First Consultation Service and guidance on medications which may be purchased over the counter.
- Patient Engagement: Handle patients' daily prescription queries (face to face, online or by telephone). Offer guidance on medication use. Support long-term condition management. Promote patient self-care.
- Prescribing Support: Work with prescribers to ensure adherence to formulary guidelines. Manage high-risk medication monitoring. Carry out audits to improve prescribing safety, quality and cost effectiveness. Contribute to the effectiveness of our medicines management team by providing support and training as appropriate.
- Administration & Documentation: Maintain accurate records. Update patient medication lists. Support compliance with CQC and NHS Regulations. Review ways of working to maximise income from PPA & QOF. Liaise with HCAs to manage appropriate stock levels for emergency use and practice procedures.
- Medicines Optimisation: Support safe, effective and cost-efficient prescribing by reviewing and managing repeat prescriptions and reducing waste.
- Continuous Improvement: Keep up to date with professional responsibilities and CPD requirements. Contribute to ongoing continuous improvement of our operations and patient safety. Respect and protect patient confidentiality and information.
