Job Overview
We are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This role plays a key part in delivering high‑quality, person‑centred care for patients living with frailty, supporting early identification, comprehensive assessment, and proactive management.
Responsibilities
- Provide specialist clinical input across community settings to optimise independence, reduce hospital admissions and improve patient outcomes.
- Act as an autonomous practitioner while collaborating with GPs, therapists, social care partners and wider services to deliver coordinated care.
- Deliver specialist frailty care and interventions, including assessment, treatment and the management of a defined caseload as part of the multidisciplinary team.
- Undertake comprehensive assessments and implement evidence‑based care plans to meet the complex needs of patients living with frailty.
- Contribute to the development of frailty pathways and support service improvement initiatives, ensuring care is evidence‑based, safe and responsive.
Requirements
- Band 6 nurse with relevant experience in frailty care or related specialties.
- Enhanced Disclosure and Barring Service (DBS) check – the Trust will cover the cost.
- Valid right to work in the UK with no need for visa sponsorship.
