An exciting opportunity has arisen at Cromwell House Community Mental Health Team in Salford. We are looking to recruit an experienced mental health practitioner to work within the CMHT. The post requires clear experience, knowledge and awareness of working within mental health in a community setting, with consideration given to those with transferable skills from other areas.

Responsibilities

  • Work proactively with service users and their carers within a multi‑disciplinary setting.
  • Takes responsibility for assessment, monitoring and management of mental health, medication and risk.
  • Hold and manage a caseload of service users and provide Care Coordinating responsibilities under the Care Programme Approach Guidelines.
  • Assess service users and carers’ needs, access and sign post to a wide range of community resources and provider services available in Salford.
  • Provide quality first‑line psychosocial assessments as part of GMMH’s Functional CMHT duty role and forge professional working relationships with multi‑agencies.
  • Work occasional weekends.
