As a Community Health Care Assistant working in the Out of Hours District Nursing Service you will be integral to the care of the patients. The Trust is focused on supporting the Community and helping patients remain independent in their own home. You will actively assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting, including supporting end of life patients.
By joining this Trust you will be coming to work in a supportive and friendly environment, with a team that are forward thinking and proactive. They are a welcoming and friendly team. The Evening and Overnight district nursing team supports patients from 19:00-08:00 across a 7 day week, 365 days a year.
Due to the nature of this post you will require a full clean driving license with access of a vehicle. The core hours of this post is 20 hours x 4 evenings per week 19:00-00:00 across 7 days covering weekends and bank holidays. You will work alongside a member of the team, working in pairs for safety with full support of your Team Leader and other members of the team.
You will provide treatment to a variety of patients with health and social care needs. This can vary from simple and complex patient care to looking after people at the end of their lives who wish to remain at home. You will work closely with the MDT (Social Workers, GP’s and Allied Health Professionals) to ensure that patient care is appropriately coordinated.
