The Health and Social Care Screening Officer post has become available within the Adult Social Care Contact Team, supporting the various Integrated Social Care Teams across the City. The Adult Social Care Contact Team is the first point of contact for people who live in Salford who may need to access social care services.
Within this role, you will be required to work closely with colleagues to provide information, advice and support to adults and their carers who may be eligible for assessment and care services under the Care Act 2014. We strive to support people to live well and to be safe at home by having strength-based conversations to promote their wellbeing.
You will be the first point of contact for the person requesting support. You will undertake strength-based conversations with people, supporting them to maximise their wellbeing. You will offer a person-centred approach ensuring a compassionate and personalised approach. You will screen and process referrals received via e-mail, electronic portal or telephone, providing ad hoc administrative support as required.
The successful applicant will have excellent customer care and communication skills with a clear understanding of customers and their needs. You will have a keen eye for attention to detail and the experience and ability to use various computerised packages including Microsoft Office applications. You will need to input accurately and efficiently into various database systems and demonstrate attention to detail when handling sensitive personal data.
You will be required to be an effective team player and be able to work on your own initiative, with excellent organisational skills, prioritising your own workload when necessary. You must have a flexible approach to work and positively embrace change.
