We are Trafford Local Care Organisation – a public sector organisation that brings together NHS community health services and adult social care services in Trafford, improving local health through cross‑sector collaboration.

Responsibilities

  • Provide a specialist patient‑centred physiotherapy service to patients referred to the community stroke and neuro‑rehabilitation service.
  • Undertake the assessment and clinical diagnosis of neurologically impaired patients using investigative and analytical skills; formulate an individual management and/or treatment programme according to the patient's goals, using advanced clinical reasoning and a range of treatment skills, including knowledge of alternative treatment options.
  • Continuously assess and evaluate treatment programmes to ensure maximum patient benefit, modifying them according to the patient's needs.
  • Assess and manage clinical risk and accept clinical responsibility as an autonomous practitioner for a designated caseload of patients, and organise this efficiently and effectively with regard to clinical priorities and use of time.
  • Utilise complex communication skills to overcome barriers to communication.
  • Provide advice and education to patients and carers in conjunction with the multidisciplinary team, and signpost to appropriate agencies and charities.
  • Stay aware of, and comply with, organisational and departmental policies and procedures, and be involved in reviewing and updating them as appropriate.
  • Maintain own continuing professional development and develop current knowledge of evidence‑based practice.
