To support individuals referred to secondary mental health services for assessment or ongoing care. As a Registered Mental Health Nurse you will demonstrate and live the NMC values and the Trust’s values of inspiring hope, working together, caring, compassion, respect, and honesty.

Responsibilities

  • Prioritising People
    • Promote positive mental and physical health to individuals under the service’s care in all situations.
    • Support individuals to collaborate in developing recovery plans, structured care plans and treatment, building a strong therapeutic alliance.
    • Assess, plan and review in line with the Care Act (2014) and Section 117 of the Mental Health Act (1983).
    • Provide clinical supervision to members of the multidisciplinary team based on clinical and professional knowledge.
    • Supervise junior staff in clinical risk assessment and formulation of crisis and risk plans.
    • Ensure all interactions and documentation are person‑centred, following NHS guidance ‘No decision about me, without me’.
    • Co‑produce individualized recovery plans with individuals, carers and others identified by the Service User.
    • Promote and maintain a learning culture within the team to support clinical effectiveness.
    • Contribute to the delivery of multidisciplinary training, including induction of new staff, students and trainees.
    • Promote awareness of and respect for equality, diversity and dignity in care, working inline with Trust and professional standards.
  • Practice Effectively
    • Demonstrate high levels of active communication skills with individuals in various forums and situations.
    • Utilise interpersonal skills to facilitate effective communication, overcoming barriers such as sensory or cognitive impairment.
    • Provide specialist mental‑health interventions in assessment, support and recovery of individuals with severe mental‑health difficulties, including assessments and monitoring of physical health and side‑effects.
    • Ensure individuals have clear and accurate information regarding their care.
    • Act in the absence of senior practitioners where required.
    • Seek out and participate in line management and clinical supervision.
    • Ensure full involvement of individuals and their carers in assessment, formulation and review of care plans.
    • Maintain high standards of clinical recording, keeping information accurate and up to date.
    • Comply with Trust policies and procedures, focusing on confidentiality, information governance and professional practice.
    • Maintain personal well‑being using available resources such as PAM assist, Occupational Health, etc.
    • Comply with health and safety legislation and protect the welfare of yourself and the wider Trust.
  • Preserve Safety
    • Identify unmet needs to support service improvement or planning, enhancing experiences of individuals.
    • Comply with relevant NMC codes, local and national professional standards, policies and procedures.
    • Adhere to safeguarding principles for children and vulnerable adults, following Trust policy and local/national reporting procedures.
    • Make safeguarding enquiries in line with Section 42 of the Care Act (2014).
    • Ensure colleagues and junior staff are competent and compliant with safeguarding legislation and guidance.
    • Follow infection prevention and control policies to maintain high standards of IPCC.
    • Adhere to service security requirements, lone working procedures and risk management, including investigations and Root Cause Analysis.
  • Promote Professionalism and Trust
    • Uphold Trust values and work in an environment free from harassment, discrimination, abuse and violence.
    • Promote the Trust’s anti‑racism agenda in all professional and personal interactions.
    • Maintain personal and professional duty of candour where incidents or accidents occur.
    • Develop and maintain good working relationships with all partners involved in individuals’ care, including GPs, commissioners and third‑sector agencies.
    • Promote awareness of mental health and the role of CMHT within local and wider community services.
    • Advocate on behalf of individuals under your care to ensure full participation in their treatment.
    • Exercise excellent time management, punctuality, consistency and reliability.
    • Demonstrate empathy and compassion to individuals or their families/carers who may have concerns about care.
    • Take responsibility for learning and professional development, using opportunities to expand clinical, professional or governance skills.
    • Act as an effective role model and practice educator for students on clinical placements.
    • Act with dignity and respect, keeping with NMC and Trust guidance.
    • Maintain professional registration, informing the Trust of any changes, conditions or restrictions.
    • Use access to clinical information only where required for duties.
    • Maintain confidentiality and comply with GDPR, ensuring information is not disclosed unnecessarily.

    • Perform any other reasonable duties as requested by an appropriate Trust manager.
    • Understand and comply with all Trust policies, procedures, protocols and guidelines.
