Hope Homecare is looking for compassionate, dedicated, and professional Registered Community Nurses to join our growing nurse‑led team. The role involves delivering high‑quality nursing care and clinical support to adults and children within their own homes and community settings, including individuals with complex healthcare needs. As a Registered Community Nurse, you will play a vital role in promoting independence, dignity, wellbeing, and person‑centred care while supporting service users safely within the community.
Main Duties & Responsibilities
- Deliver high‑quality nursing care within service users’ homes and community settings
- Assess, plan, implement, and evaluate personalised care plans
- Administer medication and monitor service users’ health conditions
- Support individuals with complex care needs including wound care, catheter care, PEG feeding, tracheostomy care, and other clinical interventions where required
- Monitor and record observations accurately and escalate appropriately
- Promote safeguarding, dignity, independence, and person‑centred care at all times
- Work collaboratively with families, carers, healthcare professionals, and multidisciplinary teams
- Maintain accurate clinical documentation and care records
- Provide emotional support and health education to service users and their families
- Ensure infection prevention and control standards are maintained
- Travel safely between service users’ homes within the community
Requirements
- Registered Nurse qualification
- Active NMC PIN
- Previous nursing or community care experience preferred
- Experience supporting individuals with complex healthcare needs is advantageous
- Strong clinical assessment and communication skills
- Ability to work independently and as part of a multidisciplinary team
- Compassionate, reliable, and professional approach to care
- Full UK driving licence and access to own vehicle preferred
Applicants must have the right to work in the UK.
Salary & Working Hours
Starting salary from approximately £32,073 per annum. Full‑time position based on 37.5 hours per week. Additional opportunities for overtime depending on service requirements.
