The successful candidate will be offered the position of Primary Care Mental Health Practitioner (independent Prescriber or ACP) on a fixed term contract of 1 year with a view to becoming permanent. The salary will be £49,000-60,000 WTE (depending on qualifications, experience, and scope of practice). They will undertake comprehensive mental health assessments and risk assessments of patients over the age of 18.

Key responsibilities include:

  • Developing and implementing evidence-based treatment plans.
  • Prescribing and reviewing mental health medications which can be prescribed in primary care according to local guidance within professional competence and excluding antipsychotics.
  • Managing a caseload of patients presenting with mild, moderate, and severe mental health conditions.
  • Providing medication reviews and monitoring treatment effectiveness and side effects.
  • Delivering brief therapeutic interventions and psychoeducation.
  • Supporting patients with anxiety, depression, PTSD, personality disorders, psychosis, and other mental health conditions including knowledge of memory and dementia issues.
  • Working closely with GPs and wider PCN teams to provide integrated care.
  • Facilitating referrals to specialist mental health services where appropriate.
  • Participating in safeguarding, clinical governance, and quality improvement initiatives.

For Band ACP applicants:

  • Providing clinical leadership, supervision, mentorship, and service development support.
