Join our dynamic adult community nursing team and make a real difference in people's lives.

As a Band 5 Community Staff Nurse you will join an Integrated Neighbourhood Team (INT) working to prevent hospital admissions, support faster discharges and deliver a high quality, patient‑centred service. You’ll work within a seven‑day shift pattern, 08:00 to 20:30, and collaborate closely with our team leader and local health and social care professionals to provide tailored care for patients with both complex and non‑complex needs.

Responsibilities

  • Lead new initiatives to reduce hospital admissions and enable quicker discharges.
  • Deliver patient‑centred care and support self‑management for a diverse population.
  • Collaborate with allied health and social care partners to provide integrated services.
  • Support the team in day‑to‑day duties including assessments, care planning, and continuity of care.

Qualifications

  • Minimum requirement: a current NMC PIN to practice as a registered nurse in the UK.
  • NHS‑registered or newly qualified nurses are encouraged to apply.
  • A current NMC PIN is mandatory; NMC PRN pins do not meet the essential criteria for this role.
