The predicted salary is between 50000 - 70000 £ per year.

Permanent contract 18.75 hours with a 2-week rota.

Schedule:

  • Week 1 – Monday 9-12:45 (Featherstone); Wednesday 9-17 (Featherstone); Friday 9-17 (Selly Park)
  • Week 2 – Wednesday 9-17 (Featherstone); Thursday 9-12:45 (Featherstone); Friday 9-17 (Selly Park)

Responsibilities:

  • Deliver medication reviews (SMR, LTCs) and complex polypharmacy management for elderly, frail, and end-of-life patients.
  • Lead person-centred medicines optimisation and local antimicrobial stewardship within the PCN.
  • Support QOF contribution and enhanced services, including learning disability/autism reviews (STOMP).
  • Integrate pharmacy teams across primary care, community, and hospital to improve patient outcomes.
  • Act as a central clinical lead in shared care protocols, specialist drug checklists, high-risk drug monitoring and clinical research of medication.

Person Specification:

  • Structured Medication Reviews, LTCs management, patient-facing role, care home medication reviews, complex polypharmacy management, elderly and frail patients care, end-of-life care pathway, QOF prescribing indicators, learning disabilities or autism reviews, specialist drugs.

Qualifications & Experience:

  • MPharm and GPhC registration.
  • Evidence of recent management and leadership development.
  • Post-graduate clinical pharmacy qualification (clinical diploma, MSc, or equivalent).
  • Independent Prescribing Qualification.
  • Willing to enrol and complete CPPE Primary Care Pathway (18-month programme) if not already completed.
  • Willing to consent to enhanced DBS disclosure.
  • Minimum 2-years post-registration experience in primary care pharmacy or 4-years in any pharmacy sector.
  • Desirable: RPS Member, ACP (Masters in Advanced Clinical Practice), familiarity with GP clinical IT systems, experience of effective partnership and working with stakeholders.
