A fantastic opportunity has arisen for an organised and dedicated individual to join our senior management team as our Care Coordinator for rota management and staff recruitment. You will ensure high-quality home care is provided to our clients by effectively scheduling care calls and supporting our care team. You will have the exciting opportunity of contributing to our continual growth and development, whilst ensuring our commitment to quality care is met at all times.
The Care Coordinator role is key to ensure smooth planning of rotas so that our home care assistants have a work/life balance and all care calls are covered. This is a fast‑paced role and requires someone who is able to work well under pressure and remain calm in a variety of situations. Previous experience as a care coordinator is preferred but not essential.
Responsibilities- Scheduling client calls, matching client’s needs to the correct carers for our home care service across South Cheshire.
- Managing holiday / absence cover and last minute changes to the rotas.
- Staff recruitment and retention.
- Supporting new staff through the induction process and probation period.
- Accurate record keeping using a bespoke IT system.
- Day‑to‑day problem‑solving liaising with clients and care assistants.
- Relationship building and communicating effectively with the existing team, clients and their families and other health and social care professionals.
- Providing care and support to clients and field based support when needed.
- To be part of the on‑call team (additional payment for on‑call).
- Ensuring all practices are CQC compliant.
- Full‑time salary of £27,500 to £29,000 plus additional payment for being on‑call (£200 per week) for a fixed contract of 37.5 hours, flexible across Monday to Sunday depending on the needs of the business.
- Pay review after 6 months.

- You must be local to the area as you will be based in our Nantwich office and will need a full UK driving licence and a car for this role to provide care as needed for holiday and emergency cover.
- You will need to be flexible in managing your working week to cover care calls when needed, which may be a morning shift from 7am or an evening shift until 10pm and may include weekends.
- Whilst previous experience as a care co‑ordinator isn’t essential for this role you must have experience in a home care role and have completed your NVQ level 2 or above in health and social care.
- We are looking for someone who is reliable and flexible to the needs of the business and able to manage competing priorities.
- You must have excellent communication, organisation and prioritisation skills with the ability to work calmly and accurately under pressure.
- You must have excellent planning and computer skills, including Microsoft office and be able to learn scheduling / rostering systems.
- Experience in CRM software is an advantage.
