An exciting opportunity has arisen within the Heywood & Middleton Community Mental Health Team for a Community Mental Health Nurse. The role involves providing mental health care to working‑age adults with moderate to severe mental health problems across a wide range of presenting conditions.
Key Responsibilities
- Deliver mental health care to patients under secondary care, including assessment, risk management, and care planning.
- Conduct evidence‑based therapeutic interventions and provide direct assistance, education, and emotional support to promote re‑enablement and recovery.
- Assess and review needs, formulate and implement care plans, monitor progress, and evaluate outcomes.
- Maintain accurate, contemporaneous electronic records in line with Trust documentation standards and Information Governance guidelines.
- Liaise with all agencies, multidisciplinary team members, and clients’ carers; provide written and verbal feedback to clients, carers, and relevant professionals.
- Carry out clinic duties such as administration of depot injections and other medication management as required.
- Participate in the CMHT’s duty system on a rota basis and respond promptly to urgent clinical needs, including crisis situations.
- Promote physical health and wellbeing and support clients in maintaining a persistent approach to engagement.
- Adhere to current legislative frameworks, including the Mental Health Act and Mental Capacity Act, and observe safeguarding procedures.
- Manage resources efficiently and maintain an accurate diary of appointments, expenses, and activity records for audit purposes.
- Engage in clinical supervision, annual IPDR process, and continuous professional development to meet professional standards.
- Participate in quality improvement activities such as clinical audit and service development initiatives.
Qualifications & Requirements
- Registered Mental Health Nurse (RMN) or equivalent professional registration.
- Experience in a relevant area of mental health practice.
- Current driving licence or supported driver with access to a vehicle.
- Strong communication and organisational skills.
- Demonstrated commitment to professional development and evidence‑based practice.
