Are you a Qualified Mental Health Nurse, Social Worker or Occupational Therapist with urgent care experience that is looking to progress and lead on the development of our Home-Based Treatment Team (HBTT)? Two exciting opportunities have arisen within Central Home Based Treatment Team to work as a Senior Practitioner (Senior Urgent Response Practitioner) within a well established team.

The post holder will take on a senior role in responding urgently to referrals from our Inpatient, Mental Health Liaison and Community colleagues, offering alternative to inpatient admission for people with complex presentations. The post holder would oversee the care of patients under the service along with oversight of patient flow to gain an understanding on where service pressures may be, and proactively support to reduce these within this role in the HBTT. The post holder will proactively support colleagues within HBTT and advocate for people to recover at home in the least restrictive way. As a senior role, there will also be a requirement to supervise and support staff members within HBTT in their professional development alongside participation in service development projects within the team and urgent care services.

Responsibilities

  • Provide senior clinical support across urgent care pathways including Mental Health Liaison Services, Home Based Treatment Team, inpatient services and community teams.
  • High Intensity User Pathway: Take a lead role in supporting the management of high intensity users and frequent attenders across urgent care services. This includes service users who repeatedly attend A&E, experience frequent inpatient admissions or present in recurrent crisis.
  • Admission Avoidance and Gatekeeping: Provide senior clinical input into admission decisions and support practitioners managing complex presentations where admission is being considered. Support inpatient teams to identify opportunities for early discharge through MDT liaison and actively reach out to service users, where ongoing care can safely be provided by HBT. Provide consultation and support to staff managing complex risk presentations including repeated self harm, emotional dysregulation, substance misuse and relational risk dynamics.
  • Staff Support and Development: Provide line management supervision, advice and reflective support to colleagues working within urgent care services.
  • Service Development and Audit: Contribute to service development work within urgent care pathways including audit, service evaluation and quality improvement initiatives.
